Interested in Bed-Based Residential Treatment?
To explore residential treatment at Holmes House, start by contacting your local addiction counseling or case management agency. ConnexOntario. Let them know you are interested in residential treatment, and they will conduct an assessment to determine if Holmes House is the best fit for your needs. If it’s determined to be the right program, the agency will complete the necessary paperwork and send it to Holmes House to add you to the waitlist.
Once the Holmes House Intake Coordinator receives and reviews your paperwork, you’ll be contacted to discuss your application. During this screening process, the intake team will go over program eligibility, current wait times, and what to expect for admission.
Eligibility Requirements
- Physical and Mental Readiness: Clients should be physically and mentally able to participate in a structured group setting with an intensive treatment program. This includes the ability to walk up to 2km each day for activities, meetings, and participation in activities of daily living. Our program takes a holistic approach to substance use treatment, focusing on the physical, mental, and emotional well-being of each individual.
- Substance-Free Period: We understand that staying substance-free for at least 72 hours before your admission date can be challenging, but we want you to get the most out of your time in our program. This means avoiding substances such as alcohol, cocaine, marijuana, crack, crystal meth, hallucinogens, non-prescription benzodiazepines, fentanyl, carfentanil, non-prescribed morphine, MDMA, and LSD. To help you feel well during residential treatment, we recommend attending withdrawal management program, a RAAM Clinic, or another similar service beforehand. - Assessment: Please complete our residential intake form, and the ASSIST Screener
- For Clients on Suboxone, Kadian, Sublocade, or Methadone: Clients must be on a stable dose for at least 72 hours. Please have your Addiction Provider fax a copy of your most recent prescription, the last witnessed dose, and a complete medication list from the past 90 days. Holmes House has an onsite provider who will work collaboratively with your provider to managing your medical treatment during your stay and you will be transitioned back to your outpatient provider at the end of your stay. Medications will be blister-packed by a local pharmacy that works with Holmes House, to ensure safety and compliance during your stay.
- 60-day medication list: Please have your pharmacy fax us a copy of your medication list for the last 60 days, Hope Pharmacy Simcoe, will be blister packing your medication during your stay at Holmes House. If you do not have medication coverage (OW, ODSP, IA, Trillium, Private work insurance) please bring money to cover the cost of withdrawal medication.
