The Haldimand Norfolk Diabetes Program

The Haldimand Norfolk Diabetes Program team provides education, counseling, and support for adults with diabetes, prediabetes, low-risk gestational diabetes. The program provides service in the communities & surrounding areas of Simcoe, Hagersville, Dunnville, with satellite clinics once a month in Port Rowan and Delhi.

Our services are free, and our open referral policy means you can refer yourself.

Our caring team of certified diabetes nurses and dietitian educators will work with you to reach your goals.   When you learn what impacts your glucose levels, what you need to know, and what you can do to manage your health: YOU set your goals

You can learn about managing your diabetes by:

  • Individual appointments (in person, phone, or virtually)
  • Pre-Diabetes Group Education Sessions
  • Library Sessions
  • Printed educational material
  • Electronic educational material
  • Monthly Electronic newsletter (Diabetes Dashboard)
