The Delhi Community Health Centre was one of 52 centres across Ontario to pioneer the Community Family Health Team (FHT) concept in 2005. Today, the Delhi Community Health Centre's multidisciplinary FHT, consisting of doctors, nurses, nurse practitioners, pharmacists and other allied healthcare professionals address the needs of a diverse community.
