Pharmacist - Part-time

Qualifications:

  • Bachelor of Science in Pharmacy
  • Licenced with the Ontario College of Pharmacists
  • Completion of hospital residency or previous hospital dispensing and clinical experience
  • Demonstrated understanding and strong commitment to the delivery of patient centered care with an eye on patient safety, including the ability to identify, prevent and resolve drug related problems.
  • Demonstrated commitment towards positive customer service when dealing with patients, colleagues, volunteers and other members of the community and public.
  • Demonstrated sound organizational and prioritization skills coupled with the ability to work well in stressful situations with a consistent and varied workload
  • Effective negotiation and conflict resolution skills
  • Excellent verbal and written communication skills, including strong documentation skills.
  • Demonstrated ability to work independently and in a interdisciplinary team environment
  • Ability to work weekends, statutory holidays and participate in on-call on a rotational basis.
  • Familiarity with unit dose, Medi-tech and Outlook systems preferred.
  • Excellent attendance record and committed to life long learning.
  • Leadership and supervisory experience an asset.

 

Principle Duties Include:

  • Providing clinical pharmacy services for the purpose of achieving positive health outcomes to improve the patients' quality of life.
  • Performing other "drug use control" responsibilities, including but not limited to, reviewing physician's orders, creating and maintaining computerized patient medication profiles, TPN, reviewing drug information and adverse drug reaction reporting.
  • Order entry of prescriptions into the Medi-tech system, clinical pharmacotherapy monitoring, and selective medications dispensing.
  • Directing and teaching both staff and patients.
  • Other relevant duties as assigned.
